[CSL board] our recent Healey grant app

The GLCF is the successful one and is maybe a better model for a 
template than the Healey.

btw, I've added a link to our "internal" documents on the board pages of 
the main website.

Right now, I'm working with a potential fundraising VISTAs, In December, 
I'd imagine creating the lucky winner's first task would be creating 
such a template for Melissa to help edit and shape.
